To be eligible to claim a Philips Sonicare electric toothbrush 2000 series, you will need to do the following:

• Schedule a consultation with your Philips Zoom! dental provider to see if you are eligible for an in-chair whitening treatment. Once your dentist gives you the go-ahead, schedule your treatment.
   

Step 2

• After the treatment, please scan the QR code on the counter card in practice and complete the online form to claim. As proof of treatment please include your receipt/invoice number and upload a clear photo of the counter card displayed.
• Click the button “Claim” in the below. Fill the claim form in by uploading the photo with the Zoom! Whitening promotion poster and submit.
  
  

Step 3

• An email confirming the submission of the claim will be sent to the email address provided.

Step 4

• Philips will ship the Sonicare 2000 electric toothbrush to the address provided on the claim form within 3-4 weeks of receiving the claim. Please note that any claims submitted after the 28th February 2026 will be declined.
