1. After a consultation appointment, book your Philips Zoom! In-chair whitening treatment.
2. After the treatment, scan the QR code on the counter card to complete the online form with your invoice number and photo of the counter card to claim.
3. Philips will provide the Sonicare 2100 electric toothbrush to the address provided within 6 weeks of receiving claim.
