How to claim your cash back

To be eligible to receive $50 cash back you must:

 

1. Consult your Dental Professional to see if you are eligible for a Philips Zoom! Whitening Treatment
2. Book in and receive a Philips Zoom! In-chair Whitening Treatment between 1 October 2018 and 28 February 2019 at a Philips Zoom! Dental provider
3. Obtain a pink Unique Code Card from your dentist at your appointment (you will need this to submit the claim)
4. Visit this page and click the below 'Claim your Cash Back' button and fill out all of the fields with your:
    

• Full Name
• Date of birth
• Email address
• Dental Practice where you received your Philips Zoom In-chair treatment
• Unique code

 

If your claim is successful, you will receive your cash back via cheque within 3-4 weeks of submitting your claim.
