1. Please call 02 9912 4490 (during business hours) at least 21 days after but within 60 days from the date of purchase (inclusive of 60th day) to obtain a RETURN AUTHORISATION NUMBER
2. Complete details on this form (also available on www.philips.com.au/promotions), including why you are returning the product
3. Send the form along with your Philips product (in its original packaging) and original purchase receipt (please retain a copy of the receipt) within 90 days of purchase (but before last mail 30/5/17) to:
